Looks like a loose print head to me.
Do a test print with a tall cylinder in the center of the bed and see if that happens again. Upload some pics when you are done. My R+ is printing fantastic after a few tweaks.

I think it is your fast few layers that look bad?
If so, this is a cooling problem. The last few layers are not getting enough time to cool, before the next layer gets layed onto it, resulting in poor printing.
You need to increase your cooling % in your software, slow down your printing the last few layers, or lower your overall print temp.
If this is PLA, you need to add active cooling, via a fan.
Now, more information would help greatly. What is the the software you are using, what is the material, what is the settings, tell us more about your printer (fan, cover, etc.) if you want more help.
